Why this exists
HIPAA requires a written agreement before a practice shares patient information with a service like HealthAgent. It protects your patients, your practice, and Bonis Systems LLC. No patient information can enter this account until the agreement is executed — the tools themselves enforce that.

- Sign below. Your name, your title, and confirmation that you're authorized to bind the practice.
